| 1. ACCOMPANIMENT |
|
Parents are encouraged to accompany their children only for the FIRST THREE days as your presence may distract your child and other children. Your child may choose to rely on you continually and the teacher will have difficulty reaching out to your child or delivering the lesson. For the children's interest, your co-operation is greatly appreciated. |

2. HYGIENE & HEALTH |

To prevent the spread of contagious diseases, we would like your child to bring a handkerchief or a packet of tissue for his personal use everyday.
Parents are advised to let their children rest at home if they are sick (e.g. having fever, cough, flu, sore eyes etc.) This will ensure speedy recovery of your children as well as prevent the virus from spreading to other children.
If your child is suffering from a contagious disease (e.g. hand, foot and mouth disease), a certified letter from the doctor to prove that the child had totally recovered must be presented to the teacher before your child is allowed to return to the centre for lessons again.
In the event of any outbreak of any diseases (e.g. HFMD etc), no refund will be entertained as makeup lessons will be conducted. |

3. TERMINATION |

A written notice is required when you wish to withdraw your child from the programme. Please state the child’s name, your name, address, contact number, reasons for withdrawal.
All payments made, including deposit, are not transferable and refundable.
Fees may be reviewed and revised periodically to meet quality of service. |

4. TRANSFER |
|
You will need to write in to our head office for approval if you wish to transfer your child from one centre to another. Transfer is possible if there is vacancy. A transfer fee of $20/- will be charged for every successful transfer. |

5. PUNCTUALITY |
|
Parents and guardians are advised to fetch their children punctually. |

Children must not wear jewelry or valuable accessories when they come for lesson.
Girls with long hair are encouraged to tie up their hair. |

7. RIGHTS OF REFUSAL |

In the event that our programme is found to be unsuitable for the child, the school shall reserve the rights to refuse and reject any application to continue the course.
The school shall reserve the rights to transfer, combine or dissolve a class. |

8. CHANGE OF ADDRESS |

Please notify the school of any change in address or telephone numbers to avoid any inconveniences which may arise. |

9. DISCLOSURE OF RELEVANT INFORMATION |
|
All known relevant information affecting the health, development of the child or needing special considerations shall be duly disclosed, failing which the school shall not be hold liable for any inconveniences which may arise. |

EMERGENCY
Teachers will contact parents or guardians immediately when there is an emergency.
If neither the parents or guardians can be reached, the teachers may:
Call an ambulance
Get a doctor
Bring the child to the clinic or hospital
Steps taken may include, but are not limited to those mentioned above.
All expenses incurred will be borne by the lawful parents of the child.
INDEMNITY
I/We, Mr / Mrs / Mdm _________________________________, being the lawful parent(s) of ____________________________________ (child’s name) hereby agree to indemnify Jenius Kindergarten and its staff against all claims, demands, actions, proceedings, judgements, damages, losses, costs and expenses of any nature which the child or parents may suffer or incur for death, injury, loss and/or damage caused, by any occurrences in the centre and/or participation in any activities in the centre which is not due to the negligence of the teachers.
DECLARATION
I/We hereby declare that the information provided above is true and any intentional suppression of relevant information may lead to the rejection of this application.
I/We have read the Terms and Conditions and hereby agree to abide by them.
In the event that one of the parents is absent to sign, one signature will be representative and binding for both parents.
